To apply for membership, please fill out the application form below (PDF format) and mail it to the address shown on the application.
If you are a current member, a renewal form will be mailed to you before your current membership expires. If you do not receive a renewal, please call the DCALS Secretary.
You will need the Adobe Reader to view the application form. The Adobe Reader is free.
